APC and retention forms


Annual practising certificate

If you wish to apply for an APC for the period 1 April 2017 to 31 March 2018, you will need to complete the 2017/18 APC application form.

Retention

If you wish to be retained on the Register, but not practise for the period 1 April 2017 to 31 March 2018, you will need to complete the 2017/18 retention application form.

Application to undertake supervised activities

If you wish to undertake supervised activities as part of your training for pursuing the Graduate Certificate of Orthodontic Assisting, complete the supervised activities as a trainee application form.

Any patients you treat must consent to this by completing the patient consent for orthodontic care by a trainee, which should be retained on the patient file.

Register as an orthodontic auxiliary

If you wish to apply for registration as an orthodontic auxiliary after the completion of either the Graduate Certificate of Orthodontic Assisting or the Certificate of Orthodontic assisting qualifications, complete the orthodontic auxiliary registration application form.

To register as an orthodontic auxiliary with other prescribed qualifications, complete the New Zealand prescribed qualification registration form.

Overseas applicant with a non-prescribed qualification

If you have completed a qualification outside of New Zealand which is not prescribed, and you wish to apply for an individual assessment of your qualification, training and experience, complete the individual assessment eligibility application form.

If you are deemed eligible to register, you will be sent a registration application form to complete.

Removal of an exclusion

If you wish to apply to have an exclusion(s) removed from your scope of practice, complete the removal of exclusion application form.

Change of Details


Change of address

If you have changed any of your addresses, please advise us by submitting the change of address form.

Change of name

If you have changed your name, please advise us by submitting the change of name form.

Restoration to the Register

If you have been removed from the Register under section 143(3), 144(3) or 144(5) of the Health Practitioners Competence Assurance Act 2003, and now wish to be restored to the Register, complete the restoration application form.

Certificates of good standing

If you wish to apply for a certificate of good standing, complete the certificate of good standing application form.

Replacement certificate

If you need a replacement certificate, or would like an additional copy of a certificate, complete the replacement certificate application form.
