APC and retention forms
Annual practising certificate (APC)
If you wish to apply for an APC for the period 1 October 2017 to 30 September 2018, you will need to complete the 2017/18 APC application form.
If you wish to be retained on the register, but not practise for the period 1 October 2017 to 30 September 2018, you will need to complete the 2017/18 retention application form.
New Zealand qualified with prescribed qualifications
If you have completed a prescribed qualification in New Zealand, complete the New Zealand prescribed qualification registration form.
New Zealand recent graduate
If you have graduated as part of the last cohort of a New Zealand prescribed qualification, complete the newly qualified graduate registration form. This form is only valid between November and March of each year.
Your application will be accepted before the graduation day, but will only be processed after the respective universities’ graduations.
If you wish to register outside of this period, you will need to complete the New Zealand prescribed qualification registration form.
Trans-Tasman Mutual Recognition (TTMR)
If you hold registration in Australia and wish to apply for registration in New Zealand under the Trans-Tasman Mutual Recognition Act, complete the TTMR registration application form.
Overseas applicant with a prescribed qualification
If you have completed an overseas dental prescribed qualification or dental specialist prescribed qualification outside of New Zealand, complete the registration for overseas applicant with prescribed qualifications application form.
Overseas applicant with a non-prescribed qualification
If you have completed a qualification outside of New Zealand which is not prescribed, complete the registration of specialist non-prescribed qualifications and experience under section 15(2) of the Act application form.
Some applications require you to submit referee reports. The referees must use the referee report.
New Zealand Dentist Registration Examination
The New Zealand Dentist Registration Examination is provided through the NDEB equivalency process.

Change of details
Change of address
If you have changed any of your addresses, please advise us by submitting the change of address form.
Change of name
If you have changed your name, please advise us by submitting the change of name form.
Restoration to the Register
If you have been removed from the Register under section 143(3), 144(3) or 144(5) of the Health Practitioners Competence Assurance Act 2003, and now wish to be restored to the Register, complete the restoration application form.

Certificates of good standing
If you wish to apply for a certificate of good standing, complete the certificate of good standing application form.
If you need a replacement certificate, or would like an additional copy of a certificate, complete the replacement certificate application form.
